SePHA AI Health Coaching Study

Start with the consent and baseline form. Scanning the flyer and completing that first step on your phone is fine. After you submit it, switch to a laptop or desktop to create your study account using the same email address, complete setup, upload your iPhone Apple Health export, and begin the full SePHA study experience.

Return to SePHA on a laptop or desktop and create your study account with the same email address.

Complete a short study setup on your computer so the platform knows who you are and what you are trying to do.

Upload And Begin

Upload your iPhone Apple Health export, wait for processing, then continue into the desktop SePHA study experience.

Step 1: Open the consent and baseline form

This opens in Google Forms. You can do that part on your phone. When you finish, come back here on a laptop or desktop and move to Step 2. The website account is a separate step.

Use the same email address in both places.

The email you enter on the Google Form should match the email you use when creating your SePHA study account.

Best device for the rest of the study

Step 1 works well on mobile. Steps 2 through 4, including account creation, upload, and chat, are best on a laptop or desktop.

What happens next

  1. 1 Open the consent and baseline form in Google Forms and submit it. This step works well on mobile.
  2. 2 Return here on a laptop or desktop and create your SePHA study account using that same email address.
  3. 3 Complete the short study setup page on your computer and move directly to wearable upload.
  4. 4 Upload your iPhone Apple Health export ZIP. Apple Watch works well, and WHOOP or Garmin only work if they sync into Apple Health first.

Data and privacy summary

The upload page explains in plain language what is deleted after processing, what remains during the study, and who can access it.

Your upload is converted into high-level daily summaries for the study. The uploaded ZIP and extracted raw files are then deleted after processing.

The remaining processed study data is deleted when the study concludes.
